New Patients

• Schedule your annual exam for the week after your period.
• Bring a list of all your current medications and supplements including dosages.
• Bring your insurance card and photo ID with you.
• Bring copies of your medical records to your appointment, especially abnormal pap test results.  
• If you are transferring from another practice, please make sure to Request Your Records. 
• All new patients need to fill out ALL of these forms: New Patient Demographics Registration, New Patient Form, Annual Consent Form and this HIPPA Consent Form prior to your appointment.
• If you are scheduled for an annual gyn exam, please also fill out this form.
• If you are under the age of 18, please fill out this New Pediatric Patient Demographic Registration.  
• All forms can be filled out and faxed to us at 855-540-2449 or emailed to us at TheModernWomanObGyn@AdvocareDoctors.com.
